Proof of Identity / Supplemental Identification
Accepted forms of Primary and Secondary IDs are listed below.
Submit a photocopy of the front and back of each ID that you present when you apply.
- Your photocopy must be on white, 8.5'' x 11" standard paper
- Black and white (no color),
- Use single side of the paper.
- Do not decrease the image size. You may make the image size bigger.
Primary ID - you only need one copy Choose one of the following (photocopy)
- State Drivers License, valid and with a photograph and issued more than 6 months ago.
- Federal, State or Municipal identification card with a photo.
- Military ID with a photo.
- INS Alien Registration Card (Green Card).
If Primary ID has been issued within the last 6 months or was issued out of state, you must provide photocopies of one Primary ID and 5 Secondary IDs.
Secondary ID
Send additional ID with your application even if Acceptance Agent says they are not required.

- Birth Certificates of your Children with your name as parent.
