Eligibility Requirements
You are a U.S. citizen and ALL of the following are true:
- Your passport is currently valid or has expired within the last five years.
- Your passport is in your possession.
- Has never been reported lost or stolen
- Your passport is not damaged. (If it is damaged, see "Damaged Passport")
- Your passport was issued when you were age 16 or older.
- Was issued in your current name. If you have legally changed your name and need to renew your passport see "Name Change Instructions".
You may request a passport renewal for any one of the following reasons:
- Your passport is expired or will expire soon.
- Your passport is low on blank visa pages.
1. Adult 10-Year Passport Book or Card
Must be a 10-year passport book or card that has expired in the last 5 years or is about to expire. The passport must be in good shape and can never have been reported lost or stolen.
2. Passport Photo
- One (1) color passport photo (2×2 with white background).

- Read the Photo Requirements! "Rejected Photos" are the top reason for Passports being delayed.
- Locate a Passport Photo Store near you.
3. Passport Application Form
Complete One (1) DS-82 passport renewal form.
Form must be completed online, generating a required 2-D barcode on the top left margin, and printed on two pages (not double sided), and signed by the applicant. Instructions pages may be discarded.
- The passport office will not accept hand-written DS-82 applications.
- Match the departure date on the DS-82 with the departure date on the travel itinerary or travel evidence.
4. Letter of Authorization
Complete one (1) Letter of Authorization with original signatures.
IMPORTANT: The Letter of Authorization (LOA) must be filled out and signed by the applicant and in black ink. The courier company name is required by the government and should be printed or written in black pen on the line above where you sign your name.
5. Travel Itinerary
- Proof of Travel departure must be within 2 weeks unless you need a visa (then proof of departure can be within 4 weeks).
- Flight itinerary (computer generated) from the airline or a travel agency, copy of airline ticket, or a corporate letter for business travel. If you are traveling by land or sea to Canada, Mexico, Bermuda, or the Caribbean, you can provide a hotel reservation, cruise tickets, or international car insurance.
Print ONE (1) copy of proof of departure
- One copy of the proof of departure should be inside the FedEx envelope.
6. US Government Fees
Include a U.S. Postal Money Order or Bank Cashier's Check for $190 payable to the U.S. Department of State.
Print name and birthdate on the memo line. No Personal Checks.
Please note: "U.S. Department of State" must not be abbreviated.

- Bank Cashier’s Check or Money Order. $190 made out to the US Department of State – print your name and birth date in memo area on Bank Cashier’s Check or Money Order.
- Letter of Authorization –signature and courier company name.
IMPORTANT: Please use the properly sized FedEx envelope, or FedEx box, to Safely and Securely Ship your Sealed Government Envelope and Photocopies.
DO NOT USE USPS or UPS. You are required to report the FEDEX tracking number we provide when you ship.
