Name Change
Eligibility Requirements
You are Eligible for a Passport Name Change if You are a U.S. citizen and:
- Your current passport is valid, in your possession, and undamaged.
- Has never been reported lost or stolen.
- You have legally changed your name since your passport was issued.
- You are able to submit your original proof of name change (i.e. marriage certificate, divorce decree, or court order).
- Your passport was issued when you were age 16 or older.
- If you are under 16 you must contact us. You will need different instructions.
1. Your most recent U.S. Passport
Your current passport is valid, in your possession, and undamaged.
Has never been reported lost or stolen.
2. Proof of Name Change Documents
You must send an ORIGINAL or CERTIFIED COPY of:
- Marriage certificate
- Divorce decree
- Court order
The document must clearly state your full name both before and after the name change.
All Original Name Change documents will be returned by the government.
3. Application DS-5504 or DS-82
If Passport is LESS than 1 year old.
- Use Form DS-5504 - If Passport is LESS than 1 year old.
- Provide verifiable documentation supporting the requested or reported amendments.
If Passport is MORE than 1 year old.
Complete One (1) DS-82 passport renewal form.
Form must be completed online, generating a required 2-D barcode on the top left margin, and printed on two pages (not double sided), and signed by the applicant. Instructions pages may be discarded.
- The passport office will not accept hand-written DS-82 applications.
- Match the departure date on the DS-82 with the departure date on the travel itinerary or travel evidence.
4. Photo Requirements
- One (1) color passport photo (2×2 with white background).

- Read the Photo Requirements! "Rejected Photos" are the top reason for Passports being delayed.
- Locate a Passport Photo Store near you.
5. Letter of Authorization
Complete one (1) Letter of Authorization with original signatures.
IMPORTANT: The Letter of Authorization (LOA) must be filled out and signed by the applicant and in black ink. The courier company name is required by the government and should be printed or written in black pen on the line above where you sign your name.
6. Travel Itinerary
- Proof of Travel departure must be within 2 weeks unless you need a visa (then proof of departure can be within 4 weeks).
- Flight itinerary (computer generated) from the airline or a travel agency, copy of airline ticket, or a corporate letter for business travel. If you are traveling by land or sea to Canada, Mexico, Bermuda, or the Caribbean, you can provide a hotel reservation, cruise tickets, or international car insurance.
Print ONE (1) copy of proof of departure
- One copy of the proof of departure should be inside the FedEx envelope.
7. U.S. Government Fees
If Passport is MORE than 1 year old and using DS-82 Form.
Include a U.S. Postal Money Order or Bank Cashier's Check for $190 payable to the U.S. Department of State.
Print name and birthdate on the memo line. No Personal Checks.
Please note: "U.S. Department of State" must not be abbreviated.

8. Call PassportExpress.us to Reserve a Courier, make Payment
Call (888) 442-4465 to Reserve a Passport Courier Appointment at the Passport Agency for your Application.

- One (1) Proof of Departure
- Bank Cashier’s Check or Money Order. Made out to the US Department of State – print your name and birth date in memo area on Bank Cashier’s Check or Money Order.
- One (1) Letter of Authorization –signature and courier company name.
